Please call our administrative offices during normal business hours (8am-4pm) at 847-202-6340. We can work with our billing company EMS|MC, to answer any questions you might have.

If you would like to obtain a copy of your personal ambulance report, you can stop by our administrative offices at 1475 N Hicks Rd during normal business hours (8am-4pm). You must bring a form of ID to verify identification.
